Sliding Scale

Healing Heart Counseling LLC’s sliding fee program is designed to reduce barriers to accessing health services by offering outpatient services at a discounted fee to those who qualify. HHC’s discount schedule of fees is based on household size and the poverty guidelines set by the U.S. Department of Health and Human Services under the authority of 42 U.S.C. 9902(2).

How to Apply

The discount schedule is updated annually based on changes to the poverty guidelines. As mandated by federal rules governing Federally Qualified Health Centers, to qualify for the sliding fee program, patients must:

• Complete the Sliding Scale Application (located in Jane).

• Provide the required proof of income for all household members.

• Confirm approval of the application and sliding scale fee from a member of the Healing Hearts team.

Eligibility

We base fees on your reported income and household size. Please use the Eligibility Calculator to estimate your household income before referring to the Determination Chart as a guide to your payment. Final fee determination will be made by Healing Heart Counseling's billing department.

All fees and additional charges are due at the time of service.
